| I hereby waive and release
Christine Grove Williams, Christine's K-9 Service,
employees/representatives of Christine's K-9 Service & the
facility where the class is being held (whether it be Furry Critterz, Shaggy Pines, etc.) from any and all liability of
any nature, for injury, damage or illness, which my dog or I
may suffer. I expressly assume the risk of any such damage,
injury or illness while attending class and while on the
grounds or surrounding area where the class is held.
I understand and agree that cancellations must be made one
week prior to the start of class to receive a full refund. I
also understand that a 50% refund is available, after that
point, only if this last min. vacancy in the class can be
filled.
